Pensacola Hospitalist Physicians LLC, a hospitalist group based in Pensacola, Florida, experienced a major data breach. Suspicious activity within the medical group’s internal network was detected on May 22, 2025. An investigation determined that an unauthorized actor gained access to internal systems and files between May 22, 2025 and May 23, 2025.
A review took place and on Sept. 11, 2025, it was discovered that the cybersecurity incident compromised both personally identifiable information (PII) and protected health information (PHI). Patient information compromised includes names, Social Security numbers, dates of birth, addresses, health insurance information, diagnosis information, provider names, dates of service and treatment information.
The total number of individuals impacted by the data breach has not been released, but may include thousands of patients from multiple hospitals. The risk of identity theft is high due to the combination of compromised personal and health information.
Pensacola Hospitalist Physicians began notifying impacted individuals by mail on Sept. 18, 2025. The data breach was disclosed to the Massachusetts Attorney General’s office on Sept. 19, 2025.
Pensacola Hospitalist Physicians’ response
In addition to required state and federal disclosures, Pensacola Hospitalist Physicians is offering impacted individuals free TransUnion Cyberscout single-bureau credit monitoring services. Patients with questions can also call 833-397-6797, Monday through Friday, between 8:00 a.m. and 8:00 p.m., EST.
If you receive a notice about this breach, from Pensacola Hospitalist Physicians or a hospital you’ve been treated at, you may want to:
- Sign up for the free credit monitoring services, offered by the medical group.
- Monitor your credit reports and financial accounts for any unusual activity.
- Be alert for phishing emails or phone calls that may use your exposed information.
- Consider placing a fraud alert or credit freeze with major credit bureaus.

The Florida Secretary of State lists the address for Pensacola Hospitalist Physicians LLC as 1000 West Moreno Street – the Baptist “legacy campus” that shutdown in September 2023. The Registered Agent is Corporation Service Company and its Manager is ApolloMD Business Services – both based in Atlanta.
